Please remember to have the following information with you when visiting our office: Your Current Address, Phone Numbers, Social Security Number, Driver's License/State ID and Insurance Information. We ask that you also bring a list of any and all medications you are taking.
INSURANCE
Our practice understands how complicated insurance issues can be to our patients; therefore as a courtesy, we will file primary and secondary insurance claims on your behalf. Our insurance plan participation includes, but is not limited to the the following carriers:
Aetna/US Healthcare, Beech Street, BCBS of GA (HMO, PPO, POS, Platinum), Champus, CIGNA (HMO, PPO), Fortis, Georgia 1st, Guardian, John Hancock, Kaiser (HMOP,PPO), MedicarE, Medicaid, One Health Plan, Principal, Prudential (HMO, PPO), State Merit, Southcare, United Healthcare (HMO, PPO, EPO, POS)
Most HMO/Managed Care plans require a referral from your Primary Care Physician to provide benefit coverage for specialist services.
